Referral to Bridge Employment

To refer to Bridge Employment’s service please download the referral form.

Then if you could complete part A and ask for a Supporting Agency (such as Social Worker, Occupational Therapist, GP or another person in a responsible position who knows you well) to complete part B and then return to Bridge Employment (see ‘contact us’ page for address).

Then you will receive a letter from Bridge Employment offering you an appointment at Bridge.

If you have any questions or need some help filling in the referral form then please ring our office on 0114 2795055 or you can come to Bridge’s Drop-In service every Wednesday from 1.30pm to 4.30pm.
